Welcome to your Parents Questionnaire For Child's Gaming Problems

Please check "YES" or "NO" in response to each question. "Gaming" means playing any electonic or video game. These can be played on computers, iPads, iPhones, Androids, Nintendo, Xbox, PlayStation, Wii, etc.

In the past year,

1a. Did your child often talk about gaming, even when he or she was not gaming?

1b. Did your child often rush off to game, even when he or she was involved in other activities?

2a. Did your child act restless, worried, angry, or sad when he or she could not game for a few days in a row? (Note: Do not include times that you would not allow him or her to play.)

3a. Did your child game a lot longer than he or she used to?

3b. Did gaming seem to be the most important part of your child's life?

4a. Did your child express an interest in trying to cut down or stop gaming?

5a. Did your child substantially reduce time on other hobbies or activities because of gaming?

6f. Did your child often not do homework because he or she was gaming?

7a. Did your child lie about how much he or she was gaming?

7b. Did your child regularly try to keep you or others from knowing about how much he or she was gaming?

8a. Did your child appear to game more often after having a bad day at school, or after experiencing a bad event?

9a. Did your child risk or lose a friendship because of gaming?

9b. Did your child risk or lose an opportunity at school because of gaming?

9c. Did your child skip school because of gaming?

Scoring: A response of "YES" to one or more items within each number (or boxed section) result in one point to the score, For example, responding "YES" to item 1a or item 1b results in 1 point; a "YES" response to both 1a and 1b also results in 1 point. Similarly, a "YES" response to one or more of items 6a through 6f leads to 1 point, but you cannot get more then one point for the items labeled with number 6.

Score range from 0 to 9. Score of 5 or higher indicate possible Internet gaming disorder.
